Source: American News, Aberdeen, S.mini storageD.Oct. 06--There's been strong interest in the federal health care program for which enrollment began less than a week ago, according to one of South Dakota's top health care providers.Getting registered takes a little doing, but the feedback has been good so far, said Pam Muller, chief administrative officer for Avera Health Plans.Avera is one of three groups -- Sanford and Dakotacare are the others -- certified to sell policies in South Dakota as part of the new federal health insurance program under which health insurance is available to all citizens. The enrollment period began Tuesday.The number of applicants has not yet been compiled by the federal government. But, according to an email from the U.S. Department of Health and Human Services, people do want to learn more about the program and are shopping."Traffic on the website and the call center continues to be high, suggesting a sustained interest by consumers in the health coverage options. Since Tuesday, healthcare.gov has received 8.6 million unique visitors, our call center has received 406,000 calls and we have 225,000 requests for online chats," the email said.HHS, though, is strapped because of the shutdown of the federal government.Not all of the folks hopping on the Internet to learn about the federal program are having luck enrolling or finding the information they need."It is slow going getting out to the healthcare.gov site and getting registered," Muller said.But, she added, there's no need to worry. The enrollment period runs through March 31, leaving plenty of time for learning about and selecting a policy, she said."And the good news is that we've received our first batch of enrollees from the federal marketplace Wednesday," she said.She wasn't sure how many people were enrolled in an Avera plan as of Friday afternoon.The state Division of Insurance can't track numbers, either, since it did not start its own health insurance exchange, rather it left that job to the federal government, said Melissa Klemann, assistant director of the division.She said her office has fielded calls with questions about the Patient Protection and Affordable Care Act in recent days, but not discernibly more than before the enrollment period started.The state operates an educational website -- healthreform.sd.gov. To enroll or shop, though, people have to visit the federal site, too.Muller said there was no good way for Avera to know what to expect when the enrollment period began because the implementation of a federal health care program is unprecedented. But she has visited with some enrollees. And, she said, those are the people who have had good things to say about the program and process.Generally speaking, Muller said, people who are on Medicare or Medicaid or who have health insurance through their employers don't have to do anything or worry about the enrollment period. But people who are uninsured, self-employed or have pre-existing conditions should take a look at options, 迷你倉he said.Each of the policy providers in South Dakota offer four tiers of coverage -- bronze, silver, gold or platinum, she said. Shoppers will likely find the costs and benefits similar, though there will be nuances, she said.Muller said that, based on what she's heard, it takes about 45 minutes to enroll online. According to a study, the average premium for the lowest-level plan for a South Dakota family of four with an income of $50,000 will be $141 a month. That's higher than most surrounding states and above the national average of $95.A state review of premium rates under the ACA is not yet finished, Klemann said.HHS said online users should notice considerable improvements to the website by Monday. Forwarded emails and Facebook posts often aren't reliable, she said, but the federal website is.Nobody will be, for instance, taken to jail if they don't enroll in the federal program, Muller said.According to the state Division of Insurance, the penalty for people who remain uninsured and do not qualify for exemptions is $94 per adult or 1 percent of the family income, whichever is greater. That fine goes up considerably in 2015.People who don't enroll before March 15, the ending date for the initial enrollment period, won't be able to do so until Oct. 1, Muller said. And that enrollment period will only last about a month and a half as opposed to six months like the ongoing session, she said.For policy providers, she said, the rollout of the program is not as much about selling policies as it is about educating people. Yet that's often our reaction to America's other first responders -- the worried friends, family and others who are the first to notice when a loved one is spiraling into mental illness but who find it difficult to get help for him.Then, when the unthinkable happens -- the way it did this week, when Miriam Carey tried to drive onto the lawn of the White House and was later shot dead by Washington, D.C., police officers -- we say, "How could no one have seen this coming?"It appears that many people did see this coming -- not the violence of Carey's death, perhaps, but her unraveling.Carey's mother told ABC News that her daughter was suffering from postpartum depression. The father of Carey's child says he called police in a panic last winter, worried Carey would hurt their infant. And Carey, a dental hygienist, was fired from her former workplace for combative behavior (though "nothing in her behavior would have led us to think this would have happened," her former boss told NBC Connecticut).By the time Carey led police on a high-speed chase through the capital, she was said to be so delusional, she believed that President Obama was communicating with her.The awful run-up to the young mother's death is wearily familiar to psychiatrist Lloyd Sederer, author of The Family Guide to Mental Health Care."I'm not at all surprised that this final, tragic event was heralded by months or longer of growing problems that were apparent to people close to her," says Sederer, who is also medical director of the New York State Office of Mental Health. He dubbed family members as "first responders" in an article he wrote for The Atlantic last year about the need for better early-warning systems in treating the mentally ill."When you unpack these episodes, what you see is that this is an often mounting course of ill迷你倉ess that, for whatever reasons, goes unresponded to -- even when family can see their loved one getting worse," Sederer says. "So the question is, how do we mobilize the right people to get someone into care and keep them in care? And how do we find the resources to enable them to stay in care?"Only 20 percent of those with a serious mental disorder get properly diagnosed and effectively treated, says Sederer. Think about that: An unacceptable 80 percent never receive treatment or services at all.Those miserable odds would improve if the stigma of needing help were reduced in the first place, says Joe Pyle, president of Philly's own Thomas Scattergood Behavioral Health Foundation.So Pyle is particularly disturbed by how the media -- in the wake of recent high-profile violent acts by mentally ill perpetrators -- now "contextualizes" these sort of tragic events to show that mental illness and violence are always linked.But the truth is, not every mentally ill person is violent, and not every violent person is mentally ill.In Carey's case, he says, "We have a woman who was distraught, who had no gun and who may been suffering from postpartum depression -- a very treatable illness -- and yet her condition yielded a violent response from the police."While Pyle isn't necessarily second-guessing the police response (once you attempt to breech White House security, perhaps all bets are off), he's nonetheless worried that this event is yet another case where mental illness was intertwined with a presumption of violence. However, experts say residents should not wait to get their flu shot, as it will protect them for the entire season. They note that the flu is always circulating and activity is already picking up in Texas.While it is recommended that everybody over 6 months old get the flu shot, experts said it is especially important for pregnant women, people over the age of 65, very young children and people with underlying chronic health conditions because of increased risk of complications from the flu."Anyone in those groups are at a greater risk if they do get the flu," said Chris Van Deusen, press officer with the Texas Department of State Health Services."If they do end up getting the flu then there is an increased risk that they can have some sort of seriouscomplications. And as we see every year those types of things can be very debilitating."Even though the flu is regarded by many as a minor nuisance, local experts said about 25,000 people die from the flu annually in the United States."The flu is not a cold ... it's an infection that causes severe upper respiratory infection, high fever and it makes people really sick," Dr. Brian Smith, DSHS regional director in Harlingen, said. "It puts people out of work for as long as two weeks, can lead to pneumonia and other complications including mortality. The flu is an important disease, and if we can prevent it that's the best way to do it."The flu is caused by a variety of different influenza virus strains, and every year a vaccine is developed based on the strains that researchers expect to be circulating, experts said.Typically a flu vaccine protects against three strains that researchers believe will be circulating. But this year a new version of the vaccine, the quadrivalent vaccine, is available."What happened in the past is that sometimes people would still get sick after they got the flu vaccine because they caught a strain that wasn't in the vaccine," Smith said. "The B strain they would have to guess at and it was about50-50 most years. So this year they went ahead and there are a couple of options that have two types of B in them, so that just increases the chance of getting protection against any strain of flu."Vaccines this year protect against the A/California/7/2009 H1N1, the A/Victoria/361/2011 H3Nmini storage strains, and the B/Massachusetts/2/2012 B strain, according to a DSHS press release.The quadrivalent vaccine also protects against the B/Brisbane/60/2008 strain as well, the release states.Still, experts said people interested in vaccinations should speak with their health care provider on a case-by-case basis.WHAT IF YOU ARE ALREADY SICK?People who have already contracted the flu experience a variety of symptoms including high temperatures, respiratory symptoms, cough, sore throat, sneezing, a runny nose, body aches, headache, chills and more, experts said.People suffering from typical flu symptoms would likely be best served by their family doctor, experts said, but any person experiencing more severe symptoms may need to go into the emergency room in case they have suffered any complications."If you are just having the typical fever, chills, body aches and cold symptoms, just make sure that you stay well hydrated and rest," Dr. Ada Cuellar, an emergency medicine physician at Valley Baptist Medical Center in Harlingen, said."If you feel that something more is going on and you feel that you are very dehydrated and short of breath, that would be a very good reason to come to the emergency department and make sure that you are not one of those people that are having a more severe case of the flu or have pneumonia on top of it," Cuellar said.Physicians may also be able to prescribe antiviral medication to patients, which can help prevent complications, local experts said."What can help is antiviral medication," Penny Hodgins, an infection prevention nurse with Harlingen Medical Center, said."That's especially important if people are in risk groups for complications," Hodgins said. "You can't get anything like that over the counter. Antibiotics are not antiviral drugs. This is a different class of drugs that are prescription medications; they can make the illness milder, have a shorter duration and they can prevent complications."Don't underestimate "abuelita's" (grandma's) chicken soup, which experts said can help alleviate the illness."Chicken soup is clinically (shown) to be effective," Smith said. "Homemade will make due quite nicely. Chicken soup in general is shown to be quite helpful. Abuelita's old recipe, that's fine. Amenities such as a 20,000 square feet clubhouse and an Olympic-length swimming pool are other features of Raffles Park."The name of the project reflects our heritage here in Singapore," said Mr Iqbal.He said Raffles Residency did not approach Raffles Park the way that local developers might have: "First of all, the developer would not spend the kind of time and money that we have already spent in preparing the site."The company has put up a show gallery for Raffles Park and staffed them with trained personnel, whereas "most Indian developers don't do that - they sell off plan".The design of the projects, the low density of homes (at four per acre) are other standou迷你倉 factors, he added. Raffles Park was also specifically catered for multi-generational living, with lifts built in for each home.On worries over India's economic trajectory, Mr Iqbal said while it is "nice" for a country to grow continually, there is always an opportunity for the right product in the right place at the right time.He noted that Raffles Park is situated in Whitefield, the heart of the IT industry in Bangalore, which is nicknamed the Silicon Valley of India, and can attract a growing number of Indian nationals who may have lived and studied abroad."So the fact is that there are always executives, senior executives, business owners who live and work in this area whom we feel would be looking for a product like this." The small number of units available also works to its advantage, he believed.Raffles Park is slated for completion in 2015, the 100th anniversary of the Jumabhoy family's launch into business.Backed by key holdings such as Scotts Shopping Centre and The Ascott Singapore serviced residences, Scotts Holdings had held over $600 million worth in assets at one point.Following a bitter feud between different factions of the family over control of the company that began in 1995, the Jumabhoys decided to sell their controlling stake in Scotts Holdings in 1998. Then-DBS Land took control of the company the following year.Since the family's exit, individual members have ventured into different businesses, including real estate.Iqbal is currently managing director and CEO at SilverNeedle Hospitality Group, which operates more than 64 properties worldwide. He has held other executive roles, such as heading the hospitality arm for Straits Trading Company and being chief financial officer at the East Asiatic group of companies. Mimi heads up furniture company WTP, while Asad is a director at Global Blue, a shopping tourism business.The three have remained close through the years."From the time I was born, we have lunch together every Sunday and that continues," Mr Iqbal said, referring to himself, Asad and Mimi.Raffles Residency's focus is mainly on India at the moment.
